Good morning. As always, my day starts with some reading. Today I am studying about some common emergency conditions like Hypoglycemia, Acute exacerbation of COPD, etc. Compared to night studier, Studying in morning hours when you have energized mind after a good night sleep is beneficial as the brain is better able to focus. You get to read in day light which keeps you alert. Silent environment and a break from social media add up your concentration too.
Hello everyone. I am Saroj, an intern doctor at TU Teaching Hospital in Kathmandu. Today I am posted to Emergency Red area. Lets get started.
We have a duty for 12 hours in a day. I will have my morning breakfast and today I am having bread omlet and tea. It is always necessary that you keep your glucose content adequate during your working hours so that you don't feel lethargic too quickly.
I am out of my hostel and it is too sunny.
Our general approach is that whenever patient arrives, we resuscitate them, do initial workup, relieve their symptoms, and ask for consultation in case its needed. Here I am filling up the investigation forms for a patient including, CBC, LFT, RFT, PT/INR, Chest X-ray, serology, Blood glucose, Blood group, USG abdomen and pelvis, ECG.
After working for few hours, its time for a breakfast and here i have got chapati and chicken curry and it looks really tasty.
Back to work again
Then, I checked all the reports of the patient so that I can discharge them if they are fine and call for followup in OPD in few days.
We have to check all patients record regularly so that patient can be discharged timely and it becomes easy to give handover to our colleagues.
It is so exhausting to work during night shift especially after 1 or 2 am. There are few patients coming at this time. At night you feel sleepy and not able to go to sleep is too hard to control. I asked one of my colleague to keep eyes on my patients too. To make myself feel little energetic, I walked here and there in our garden adjacent to our emergency builiding.
After returningI started arranging files according to their consultation needs so that it becomes easy to call on duty doctors from different departments.
Since we have a separate cardiac emergency centre , we generally don't get many cases of cardiovascular emergencies.

Talking about the procedures that we do as an intern are like putting Foley's cathethers, draw blood, start fluids, ascitic tapping, pleural tapping, Arterial Blood Gas sample, conduct CPR and other procedures.
Working as an intern is a satisfying job, and your satisfaction gets double when you are working especially in Casualty as you are in charge of your patients, you have to take decisions and you are making a difference to someone's life. This is what you wanted to do as a doctor. So, it feels great working in a casualty.
